What this program does...
The St. Joseph’s Healthcare Hamilton Centre for Diabetes Care and Education provides assessment, education, treatment and support for for people at risk of developing or living with diabetes (Type 1 diabetes, Type 2 diabetes, pre-diabetes, and gestational diabetes). Our goal is to provide tools and techniques to manage diabetes and live a healthy lifestyle.
We are committed to ensuring that people with diabetes have all the resources and support they need to care for themselves with confidence.
Diabetes programs are offered at both our West 5th Campus and King Campus locations.
Referral Process
For one-to-one appointments with members of our diabetes team, your family doctor can send a referral, or you can refer yourself. Our diabetes care team works closely with you and your family doctor. Without a referral, you can refer yourself for a one time visit with a Registered Dietitian or to one of our classes.

How this program helps...
You will learn about what diabetes is and how to manage it and care for yourself. The program is set up to help you meet all your educational needs. We offer one-to-one teaching and group sessions.
One-to-One Appointments:
- Meet with the nurse educator and registered dietitian, who are certified diabetes educators.
- Meetings are spread over several months to give you time to apply what you have learned.
- During these meetings you will receive information about blood sugar testing, medications, activity, nutrition, managing stress and preventing potential problems.
- You will be able to talk about your concerns and ask questions.
Group Education Classes:

LIVING WELL WITH DIABETES
For the individual newly diagnosed with diabetes, prediabetes, or as a refresher, this class provides a general overview and helpful hints on how to live a healthy life with diabetes.

CARBOHYDRATES
Everything you wanted to know about carbohydrates and more. Learn which foods have carbohydrates and how counting carbohydrates can help control your blood sugar.
All classes are held at St Joe's King Campus. Classes are open to the public, you do not have to be an existing patient at St. Joe's Diabetes Program to attend classes*.
*All persons interested in attending class must call to register as all classes are subject to enrolment. Contact us (below) to register.

- Registered Nurses who are Certified Diabetes Educators

Preparing for Your Visit
Please come prepared for every appointment by bringing:
- OHIP Health Card, and/or your private medical insurance plan information. If you do not have OHIP or private coverage there is a hospital facility fee.
- A list of your current medications, preferably from your pharmacy, including non-prescription drugs, such as vitamins and supplements.
- Blood Glucose logs, Glucometer, or Libre reader or online access to continuous glucose data.
- If you use an insulin pump, please upload it prior to your appointment.
- A family member or support person if you wish.
- Low blood sugar supplies (glucometer and treatment).
Please give 48 hours notice to cancel or change an appointment.

The Diabetes Program is located on the lower level. Turn left out of the elevator and follow the signs to the Diabetes Program/Health for Adults Program.
West 5th Campus
The Diabetes Clinic is located on Level 0, Medical Outpatient Clinics
Registration Procedures
Please bring your health card. You will register at the booking desk.
After Your Visit
If you are living with prediabetes or type 2 diabetes and you have learned about diabetes and your blood sugar is controlled, you will return back to your health care provider for care. You can come back to our program if your condition changes. If you are living with type 1 diabetes, our team will continue to provide ongoing care.
