There has been recent media coverage regarding Day Clinic closures, specifically the Centre for Diabetes Care and Education. To ensure there is clarification within the community we have provided additional information about the Centre for Diabetes Care and Education, and the services it continues to provide.
The St. Joseph’s Healthcare Hamilton Centre for Diabetes Care and Education is a vibrant outpatient service that is offered at our King Campus and West 5th Campus locations. The program serves and supports individuals with Type 1, Type 2, insulin pump, gestational, and complex diabetes needs.
Our goal is to provide the tools and techniques required to help patients manage diabetes and live a healthy lifestyle. Patients learn about what diabetes is and how to manage it and care for themselves with confidence. The program is set up to help meet educational needs offering one-to-one teaching and group sessions. Appointments can be self-referred and are Monday to Friday, with evening availability to facilitate various patients’ schedules.
The Diabetes Care and Education team includes certified diabetes educator nurses, certified diabetes educator dietitians, social workers, nurse practitioners, exercise specialists, and Endocrinologists are available to provide care for complex patients.
We are committed to ensuring that those living with diabetes have all the resources and support they need.
